You can register as a new patient online using the forms below.
Once you have submitted the forms you will need to email us your proof of ID. Our email address is: och.surgery@nhs.net.
We accept the following: photo ID (passport or driving licence) and two proofs of address (Council Tax or Tenancy Agreement) and a utility bill (gas/phone/electricity bill, NOT bank statement) with your name on it.
We can only complete your application once we have received your ID documents via email.
We invite all of our new patients to have a New Patient Health check, which means an appointment with a Health Care Assistant. Please book this at your registration if possible.

Out of Area
Recently there has been much publicity about the new legislation where patients can register with any GP of their choice. At the moment NHS England have not fully implemented all the support services for this and therefore The Old Court House Surgery are following BMA advice whereby we cannot accept patients who do not live within our boundary which is advertised in the Surgery.
Practices have a professional responsibility to ensure patients have access to home visits if they are too ill to travel to the Surgery and District Nurse services where clinically appropriate, and until NHS England assure us that patients are able to access these services regardless of where they live we cannot accept out of area patients.

A Named Accountable GP for all Patients
Your Named Accountable GP is responsible for the overall care provided to you at this Practice. They will take lead responsibility for the co-ordination of any health services that may be required.
For Patients aged 75 and over the Named Accountable GP will also work with relevant associated health and social care professionals to deliver a multi-disciplinary care package that meets the need of the patient and will ensure that you have access to a health check.
